Together TakeMeHome, in collaboration with Emory University, is working in partnership with ShipBob for our shipping and fulfillment. We transmit your name and address to ShipBob, and store your contact information in our secure databases for the purposes of sending and monitoring the status of your order, and for follow-up email and text communications, if you opted to receive these communications.
Our HIV self-tests are shipped by ShipBob. They arrive in an unmarked brown box with a label that does not list our program’s name or HIV test. The return address of the send is listed as TTMH, 46 Isidor Cr. Suite 101, Sparks, NV, 89441 or TTMH, 2201 Green Ln Unit 3, Levittown, PA 19057.
We may disclose your personally identifiable information (e.g., name and contact information) regarding your order to your state’s Department of Public Health for public health activities related to preventing and controlling the spread of disease. Further, we will disclose information regarding your order with the U.S. Centers for Disease Control and Prevention (CDC), which is covering the cost of the program and kits. Your personally identifiable information will not be shared with CDC. The CDC will use your order information to plan their HIV testing and prevention services.
